You have pretty much free range on what goes on the card.  I just need something simple to hand to an owner, simple instructions on how to obtain a license.  The card is like a playing card, 3.5" x 2.5" .  Both sides, one Davenport, the other Bettendorf.  Look through the instructions and sort it to make it simple and easy to read.  Thank you.
