Web Design Brief

Danco Transmission & Auto Care www.godanco.com needs a 1 page information gathering site where contest entrants will be directed after clicking on a banner we have already designed. We need this page to collect their Name, Address, Phone, email, and year make and model of the vehicle they drive. It must be simple and clean looking and hold their interest so they actually sign up. On the back end, I would like this information to be collected placed into an excel file and emailed to us one time a day. We do not have much time so Fluent English is a must. I have supplied our current URL because I would like this page to have the same look and feel as our current webpage using our red white and blue colors and our logo.
