1. Company Name: Jaqua Chiropractic (pronounced Jay Quay)
2. Describe the type of business you have i.e. what type of products or services your company supplies… etc.: This is a new chiropractic clinic. Our main service is spinal adjustment, but we also offer other pain relief and wellness services such as laser therapy, ultrasound, gua sha, athletic taping and more.
3. What type of overall mood would you like to project with your new logo? The overall feeling would be that of a high-end health spa. Our interior will be much like an inviting spa or relaxing massage spa.
4. How would you describe the effect you are looking for in your logo:
o Cutting-Edge
o Unique/Creative
o Clean/Sample
o Classy
o Professional
o Modern
5. Would you like a symbol or a logotype?
o At first, I was thinking a symbol would be best - something that might include an abstract symbol of a spine (a simple curved line?) or even incorporating the curve of the spine into the letter "J" in Jaqua. I would eventually like to use the symbol (or the "J") on its own, without "Jaqua Chiropractic" - but that will be later down the road when our name is a bit more established.
o And, since the name Jaqua is similar to the high-end automobile Jaguar, I thought it might be nice to use a similar font as the one Jaguar uses. Additionally, I think it would be best to treat JAQUA with all caps because of the letter Q. Otherwise, people confuse the lower case Q with a lower case G.
6. For which categories will your logo will be used?
o Web Site
o Printed Materials [Business Card, Stationery, etc…]
o Embroidery
7. What colors would you like to see in your logo? I'm not completely sure about colors. I want the font to coordinate with our office interior colors, but I have yet to decide on those. So, I might match our interior to the logo. Burgundy is a no. I like cobalt blue and sage or olive green. Browns and beige are good, too. But, I'm open to looking at completely different color schemes.
8. How many colors do you prefer in your logo?
o I know 2 is better for printing, but that limits creativity. I think I’d prefer full-color.
o I will also be using the logo to embroider on shirts and scrubs for the clinic.
o Additionally, I need the logo as a transparent image, black & white and full color
9. Please state any preference of colors/color combinations you would want the designer to use in your logo samples.: cobalt blue/sage green, other blues/greens
10. Please state colors you would NOT want your designer to use in your logo samples. NOT burgundy
11. Do you have font style preferences? [Corporate, Script, Bold, Light etc]. I like classy fonts such as the font used in the Jaguar logo.

I also wanted to add that I would love to see not only a logo with JAQUA CHIROPRACTIC on one line, but some with JAQUA on one line and CHIROPRACTIC underneath.
