T-shirt Design Brief

I need a tee-shirt designed for my gym.

On the front pocket I would like the number 5 with the text
"CrossFit MASS" "5 years strong" and " "Serving the Valley since 2009"

The back is a shield or 'crest' quartered to contain, Olympic rings, weights and elements of the U.S. flag. The text across the top should read "CrossFit MASS"
and around the 'shield' should read “Knowing is not enough; we must apply. Willing is not enough; we must do.” The gyms url is "crossfitmass.com" so if the that address could be worked in somewhere that would be great.

I have rough sketches of what the front and back should look like that I will attach.
