Graphic Design Brief

The image mockup is attached. We’re hoping to create two versions, one as a standalone with the information below and the second one a template that people can add their own examples to (we have a running list of examples). Where it says elder in the trunk we are hoping to have a realistic face in the trunk of the tree.

This visual representation brings the 4Ms of Age-Friendly Health Systems—What Matters, Medication/Medicine, Memory and Thinking, and Movement—into a culturally rooted framework that honors and uplifts American Indian and Alaska Native Elders.
Inspired by The Sacred Tree and Indigenous teachings, the tree at the center symbolizes the strength, wisdom, and resilience of Indigenous Elders. At its heart is the Elder, whose face in the trunk (attached are examples of images of elders faces) represents the living spirit and center of the community. Their knowledge and values are deeply rooted in language, land, sovereignty, culture, traditions, and history, which nourish their well-being.
Each branch and leaf reflects aspects of the 4Ms—nurturing not only clinical aspects of health but also the spiritual, emotional, and communal dimensions that are vital to Native health and aging.
4Ms Cultural Interpretations in the Image:
What Matters: At the top branches, we emphasize the importance of listening to each Elder’s voice—what they want for their physical, mental, emotional, and spiritual well-being. Examples include: 1) Family 2) Independence 3) Joy and Happiness
Medication / Medicine: On one branch, we reflect a holistic view of healing that includes traditional and Western practices. Elders’ safety and alignment with What Matters is central. Examples include: 1) Regular medical care 2) Traditional medicines 3) Prescription medications
Memory and Thinking: Healthy minds are essential for Elders to carry out what is meaningful. This section includes examples like: 1) Sharing stories 2) Keeping appointments 3) Social connection
Movement: Safe, supported movement keeps Elders active and engaged. Examples in this section include: 1) Activities of Daily Living 2) Living at home 3) Exercise
Roots of Wellness:
Beneath the tree, the roots ground the Elder in:
-Indigenous Knowledge
-Language
-Land
-Sovereignty
-Culture
-Traditions
-History
These roots are the foundation of strength and resilience, and they support Elders in living out What Matters most.

Versions of the Image:
Standalone Graphic: This version includes the complete tree with all key examples of each 4M domain and rooted cultural values.
Community Template: A customizable version for local use, allowing Tribes and organizations to insert their own community-specific examples under each branch and leaf, ensuring it reflects their own Elder priorities, medicines, and traditions.

We will need to have a funding statement at the bottom but we are still waiting on it so please keep space for that.
