In the United States, we face a paradox: despite possessing extensive knowledge on how to improve health and well-being, life expectancy is declining due to preventable diseases and even further shines a bright light on the challenges with inequity in health.
This alarming trend reveals a critical truth: knowledge alone is not enough. The lack of equitable, efficient, and knowledge-based approaches to improving public health has eroded public trust and inhibited strategies to improve health equitably for all people in all communities. We need to translate knowledge rapidly into action to create a real-time and continuous cycle of learning – intervention - impact.
PRISM (Population Research, Innovation, and SysteMs for health equity) is a groundbreaking initiative to revolutionize population health. PRISM will launch the first-ever learning population health consortium, seamlessly integrating healthcare systems, dynamic wearable and patient-generated data, public health departments, and community networks into a unified, dynamic learning system.
Real-time learning and action: PRISM will continuously gather evidence, implement interventions, and monitor health outcomes across diverse communities in real-time. This rapid cycle of learning and action allows for immediate adjustments and improvements. By fostering a culture of learning, PRISM aims to achieve measurable improvements in the health of individuals and communities across the US, heralding a new era of population health innovation.
Led by Northwestern University jointly with the University of Florida, the Louisiana Public Health Institute, and Patient is Partner LLC, the consortium includes health systems and communities across the United States including Vanderbilt, Mayo Clinic, Essentia, OCHIN, Marshfield Clinic and Research Institute, University of Missouri, University of Nebraska, University of Pittsburgh, Johns Hopkins, University of California at San Francisco, Ochsner, Baylor Scott & White, Duke University, University of Illinois at Chicago, University of Colorado, University of Utah. PRISM works at all levels to drive both local and widespread changes to attain health equity in communities and diffuse impactful innovations across the nation. PRISM’s unity with people and communities will regain public confidence in effective strategies to improve health and wellbeing.
