I think you may have misunderstood the ask. This is not an employee survey tool. It’s one employee asking colleagues for feedback. What you have provided seems more suitable for a survey monkey type surveys

Please note that I dont need a Login screen or a profile screen. The screens i have asked for will sit inside an application that already exists.....
