Flyer Design Brief

We are a high end Tanning Salon that offers multiple levels of Tanning, Red Light, Sunless, and Hybrid (UV and Red Light in a single session).

I want to modernize our current brochure (the original is included in the links below, along with some changes to it), and add a sign-up form for new clients that helps guide them through the purchasing process.

I want the sign-up form to be 11 x 17 double sided, with the demographics and purchasing information on one side, and the waiver/terms on the back side. This form needs to be super clean and easy to fill out, and should lead the client through the sign-up and purchasing options. Without knowing how this might look or how much space it will take, I do anticipate revisions after the first draft.

The information provided in the links isn't formatted as good as I would normally like, but you'll obviously keep the layout consistent. There will be some images needed after the first draft.

Once done, I will probably need these in various formats (print and digital) with any necessary revisions to accommodate the various formats and uses.
