Word Template Design Brief

I need a simple meeting protocol (as a coach) where I can enter what was discussed with the client directly in Word after a client appointment, with date, participants and location.

The meeting minutes should be a fixed design, which means that I need a place for my logo and the rest, where text is to be inserted, needs to be editable.

I have uploaded 3 postponed ideas below.

There is also a field where I can sign.

The template should be used in Word, have a modern design and be target-oriented (no frills).
