Hi,

I need a new design for my patient fact sheets. I am moving from my current clinic and I am looking from a new look.

I have attached
1. a current Fact sheet
2. New home page to the website
3. New Logo.

They need to be single page and easily readable for patients.

The will be in both PDF and print form in the future.
