Flyer Design Brief

I am making a journal for tracking pain symptoms for TMJ (Jaw) pain. I have the cover designed and I just need the journal page designed that will repeat throughout the journal. The journal is the image on the left of the bundle. The page I would like to be developed is the picture that I hand sketched out. It should be a 6 x 9 page, similar to the attached example. Greyscale, no color. Thank you!
