Graphic Design Brief

Hello,
I currently run a Speech-Language Pathologist website and have created a planner for my fellow Speech Therapist. I would like someone to create 5 different planner covers so my followers will have different options to choose from.
Here is my planner from last year to give you an idea of what the planner will look like http://www.speechtherapystore.com/free-slp-planner/

My followers are mostly women aged 25-50. However, I don't want the covers to be overly girly or excessive with flowers. Simple is better.

I'd also like there to be an editable text box to allow the Speech Therapist to type their name on their cover if they wish.

Planner Cover can say any of the following: SLP Planner, Planner, Speech Therapy Planner, Speech-Language Pathologist (maybe this title could be editable as well??).
