Hi,

I think overall it's too plain. There's not enough going on around his head to symbolise mental distress. He's also straight on when I have specified for him to be at an angle. I don't like the fact that he looks like something straight from a stock photo website.

I misunderstood an angle, I thought it's angle for head to look down. Sorry.
I can change the shape around the head to more symbolise mental distress.

I'll send you redesign shortly.
