Print Design Brief

We need a front cover 1st page and back cover 6th page designs for a printed 8.5 x 11 booklet (describes our professional services for physicians - see attached document). We basically perform back-office work including medical claims submission, insurance follow-up for outstanding claims, and patient collection services through superior software & staff solutions.

Requirements

Must have

• Address, phone and website information should be somewhere on the back cover page, while at least the front cover should have our logo incorporated somewhere (see both attached files).
  
  

Nice to have

• For the front: I'd like to see something either slick, professional & abstract that complements our colors or a clever photo/illustration that demonstrates the complexities of running an office and how we can improve their workflow & results.
  
  Back: Less is more... basic information and perhaps one illustration.
  
  (see the Folder example attached)
  
  Would be nice to have a footer and/or header design element that spans across both front and back.
