Letterhead Design Brief

Hello,
We are looking for a custom order form/receipt book that we will give our customers when they come into our office and make a payment to us as a deposit for an apartment they wish to rent.

I plan to upload the design from here onto a website that will print them in a receipt book.

I am open to all different size receipts it just must have our information and preferably our logo.

I attached our logo, and two different receipts/forms we have used in the past. There are good aspects of both but neither is what I'm looking

Requirements

Must have

• It must have the word "Receipt" at the top
• It must have a line/box for "Date"
• It must have a line/box for "Received from"
• It must have a line/box for "Address of Apartment"
• It must have a line/box for "Agent" (we will fill in which real estate agent the people are working with on this line)
• It must have a line/box for "Amount"
• It must have a line/box for "Memo"
• It must have a line/box for "How Paid" ...the options should be cash, check, or other
• There must be a disclosure that there is a $25.00 application fee per person somewhere visible
• It must have the following disclosure followed by a signature line:
• "I hereby deposit monies under the following conditions; If application is not approved or accepted by the owner or agent, the deposit will be refunded, waiving any claim for damages by reason of non-acceptance by the owner or agent. This deposit is intended to lease rental unit noted above, and for RentProv Realty to verify application data. If you decide not to take the apartment before the application status is received, the deposit is non- refundable. If the application is approved by the owner or agent, the deposit is non-refundable. RentProv Realty charges a $25.00 application fee per adult applying for the apartment. If a refund is necessary, the application fee(s) will be deducted from the refund.
