I need a pricing card with the different membership options on it.

The membership options are;

12 Month Minimum Term:
No Joining Fee, $25 Access Card Fee, $12.25 per week

No Contract:
$89 Joining Fee, $55 Access Card Fee, $13.50 per week

Pay in Advance:
12 Month - $540
6 Month - $295
3 Month - $190
