Flyer Design Brief

I need a double sided DL size (99mm x 210mm) brochure, promoting a doctor seeing patients in a new area.

I want something professional. I have included the relevant information. I would also like the Dr's picture somwhere in the brochure, with a professional background. I wish to print these tomorrow, so i will pick a winner within 14 hours of posting this ad.
