Hi, thanks for all your designs.

As I have pointed out on some feedback messages I would probably prefer a version without my head profile.

Also with the operating locations, you can take out the "hospital" next to each location

Also, my secretary has informed me of two things:

1. There needs to be horizontal blank space on the back of the card filling up HALF of the whole vertical distance of the back so that a broad but narrow sticker can be placed

2. The consulting locations each need an empty tickbox next to them

Although I like the black background colour of many of the designs I will most likely go for a white or lighter background to fit in with the referral pad design & website etc.
