Packaging Design Brief

I am looking for a logo and packaging design for my new company, "KaddyBox". KaddyBox is monthly subscription box that delivers golf equipment and accessories directly to the customer's door. The box will be a standard craft material Roll End Front Tuck mailer - the dimensions of the box are as follows:

9"x12"x3.5" (LxWxH)

I would like to see a simple and modern design that ultimately communicates innovation and creativity with the sport of golf. The box will be standard craft cardboard material therefore I would like to incorporate colors with good contrast; I am visualizing black/gray/white but am open to other options as well. If possible I would also like to incorporate the following slogan in some way:
