Print Design Brief

We need an editable curriculum card design for a martial arts system. Would like to have our logo in background or highlighted on card. Each level will have their own separate card that can be printed onto a postcard sized paper (5x7 and 6x9). Each technique has three parts to it E, F, and S and would like them to be able to have a line or box which an instructor can initial or check next to each technique. Would like to have each curriculum card be straightforward with slight modern and futuristic touches similar to the emblem and logo.
