Great thanks, time has been added to the deadline as well.

With the 2 that have been submitted so far.  I like aspects of each of them.  I really like the boldness and the contemporary feel of the one with the(great job with using the logo in the background of the order form and I like the colors and curves.  What it is missing though is maybe a smile/part face.  The bullet pointed benefits of the product.  Maybe an "Order Now" stamp by the free sample.  Also it should have that the cleaner is for all Teeth Night Guards, Teeth Retainers, Dentures, Teeth Partials, Sport Mouth Guards and Teeth Whitening Trays.  Lastly the brochure should be a standard 8.5x11'''' tri fold.  I thought maybe the benefits bullet pointed could be placed above the sample?  Also, please note that we mainly sell teeth night guards(www.teethnightguard.com) and I added a picture of a night guard...maybe it could be used on the brochure with a shine behind it or something to signify that it has been cleaned/disinfected by the product(might be a cool effect)?

For the other design, I really like it, could the design have more of a contemporary look to it?  It has some of the items mentioned that are missing from above, but is also missing a few.
