You want form designed or iPad app ?

Why do you require at least 5 pages for design?

The form needs to be designed for input iPad screens. I don't need a full app designed, just the input screens for this form. This form is one part of an app that already exists we are just having trouble getting this form into a digest able layout. Please let me know if you have any other questions!!

It doesn't have to be 5 pages, just as many as needed so that the screens aren't overloaded with info. We don't want more that 2 sections on 1 page. Please let me know if you have any other questions!! Thanks!
