Book Cover Design Brief
I am an independent author writing a book about my life in the military while serving under the Don't Ask, Don't Tell Policy. The book will be published in both print and E-book format. For the print book, the front cover should be military dog tags with the main title printed on the tags while the subtitle printed at the top. The bottom portion of the front cover should have my name, "Carlos G. Coronado". For the spine, the main title and my name should appear. The back will be reserved for the book's teaser as well as web contact information, both of which I will provide at a later time (still in development). The E-book cover can simply be the same as the front cover design for the print book.
Updates
Update! Change of heart on the positioning of the main title and the subtitle. Prefer the main title above the subtitle.

Thanks for all the submissions thus far. Lots to choose from! I finally have some info for the back covers. If you have time, could you include the following on the back.

Nice to have
