Web Design Brief

I need an HTML map that I can embed on my web page. It needs to be a map of Michigan that shows boundaries for each area code. Each of these areas needs to be clickable and link to a specific URL (I will supply the list of URL's that each area will link to. The map needs to be mobile-friendly and easy to embed on any web page. I have attached an example of what the map should look like. Each colored section on the map should be an independent, clickable link. Show counties and cities on the map but not the area codes.
