Landing Page Design Brief

We need 3 lead generation landing page templates for users looking for skill based training and degrees. Landing pages should focus on users taking action and showcase juicy imagery of their future (i.e. becoming a nurse, hvac mechanic or business degree). Some of our users have dreamed of becoming a nurse, some have just realized that they're in a dead end job and looking for a change. The landing pages should convey an uplifting message of reaching your goals and changing your life.

Technical Requirements:
Pages do not need a menu and should have minimal exit points. A phone number and call to action are required but should be secondary to the form. Must include mobile responsive design.

Landing pages should incorporate a short form with the following fields, plus a space for disclaimers below the submit button:
