I have put together a final wireframe / prototype of what I would like:

I would like this designed / branded to fit the theme of www.blabberjax.com (i.e. color, fonts, etc) and also with a similar feel to ([Non image link removed]
Final design does not need to look exactly the same as the mockup linked above, but it should have all of the same elements.
